Too often are these two parties excluded in decisions and planning for the treatment. We believe communication is key and that including the plan managers and support co-ordinators in the decisions will allow for better continuity, allowing for smoother transitions and the patient getting the most out of treatment. We often hear of both plan managers and support co-ordinators being left in the dark when it comes to:
